Sevelamer carbonate is a non-absorbed phosphate-binding polymer free of metals and calcium. It binds phosphate in the gastrointestinal tract through ionic and hydrogen bonding, reducing phosphate absorption and lowering serum phosphorus levels.

✔️ Dosage & Administration
  • Starting Dose:
    • 2.4 g (Three 800 mg tablets or sachets) or 4.8 g (Six 800 mg tablets or sachets) daily, taken three times a day with meals.
  • Titration and Maintenance:
    • Adjust dose based on serum phosphorus levels every 2-4 weeks until target levels are achieved. The average daily dose is about 6 g.
  • For patients switching from other phosphate binders:
    • Adjust the dose on a gram-for-gram basis and monitor serum phosphorus.

Swallow intact; do not crush, chew, or break.

  • Pediatric: Not recommended for children under 18 years. Limited effectiveness was noted in children <13 years and those not on dialysis.
  • Geriatric: Insufficient data for patients aged 65 and over; no significant differences noted, but cautious dosing is recommended.

  • Ciprofloxacin: Avoid simultaneous use; can decrease ciprofloxacin's bioavailability by about 50%.
  • Other Drugs: May affect absorption of ciclosporin, mycophenolate mofetil, tacrolimus, and levothyroxine. Monitor levels closely if used together.
  • Anti-Arrhythmic and Anti-Seizure Medications: Exercise caution; these were not studied in clinical trials.
  • General: Administer other medications at least 1 hour before or 3 hours after Sevel to avoid reduced bioavailability.
